FY 2011 Self-Help Homeownership Opportunity Program (SHOP)
Market Survey Documents

Thank you for completing the HAC FY 2011 SHOP Market Survey. To complete the process, please submit the following documentation by no later than 11:59 p.m. on Monday, August 8, 2011:

• Participation Commitment Letter – please carefully read the Participation Commitment Letter Instructions before completing the letter
• Leveraging Commitment Letters/Documentation that supports leveraging indicated as “firmly committed” in your Participation Commitment Letter
• Form HUD-2990, Certification of Consistency with the RC/EZ/EC II Strategic Plan (if applicable)
• Form HUD-2995, Certification of Consistency and Nexus between Activities Proposed by the Applicant with Livability Principles Advanced in Preferred Sustainability Status Communities (if applicable)
• Authorization to Incur SHOP 2011 Eligible Costs form for incurring SHOP-eligible costs (e.g., land acquisition and infrastructure improvements) on or after July 6, 2011, the publication date of the FY 2011 SHOP NOFA, that will be submitted for reimbursement, if affiliate application
