Market Survey Documents

Thank you for completing the HAC FY 2012 SHOP Market Survey. To complete the process, please submit the following documentation by no later than 4:00 p.m. on Wednesday, March 28, 2012:

  • Participation Commitment Letter – please carefully read the Participation Commitment Letter Instructions (included) before completing
  • Leveraging Commitment Letters/Documentation that supports leveraging indicated as “firmly committed” in your Participation Commitment Letter
  • Form HUD-2990, Certification of Consistency with the RC/EZ/EC II Strategic Plan (if applicable)
  • Form HUD-2995, Certification of Consistency and Nexus between Activities Proposed by the Applicant with Livability Principles Advanced in Preferred Sustainability Status Communities (if applicable)
  • Authorization to Incur SHOP 2012 Eligible Costs form for incurring SHOP-eligible costs (e.g., land acquisition and infrastructure improvements) on or after February 27, 2012, the publication date of the FY 2012 SHOP NOFA, that will be submitted for reimbursement, if affiliate application

Please note that this is not an application for SHOP 2012 funds. The SHOP 2012 application will be released on Monday, April 2, 2012 with a return date of May 21, 2012.
